Time-Out

Today we “PinPoint”: Time-Out

Must be performed prior to starting procedure (surgery or bedside procedures)

 

All those involved in procedure must participate

 

The following should be reviewed in the “Time Out”:

–    Correct patient identity

–    Correct procedure

–    Correct side and site are marked (if applicable)

–    Correct position

–    Safety precautions based on patient history

–    Required equipment, devices, medications, images, & results available

 

Must be documented in EPIC in the procedure note

 Examples of bedside or outpatient procedures that require a Time-Out

•      Amniocentesis

•      Circumcision

•      Endometrial, vulvar, vaginal biopsy

•      ROP (retinopathy of prematurity) Laser Treatment

•      Burr holes

•      EVD (External Ventricular Drain) Placement

•      Sigmoidoscopy, colonoscopy, endoscopy

•      Any procedure requiring moderate sedation

•      Central Line Insertion

•      PICC line insertion

•      Chest tube insertion

•      Lumbar puncture

•      Paracentesis

•      Thoracentesis

•      Arthrocentesis

•      Bronchoscopy (interventional or diagnostic)

•      Tracheostomy
