The Cooper Urban Health Institute team recently celebrated the integration of Cooper Family Medicine at the Kroc Center into the Urban Health Institute. Steven T. Kaufman, MD, Medical Director, and Kathleen Stillo, MBA, Executive Director, hosted an open house on Wednesday, June 21, to introduce new staff members and provide tours and information.
It was four years ago that the Cooper Advanced Care Center (CACC) opened at the Sheridan Pavilion, Camden, with an exciting new patient care model. The CACC’s innovative models of care addressed social determinants of health and improved patient outcomes at lower costs.
Patients of Cooper Family Medicine at the Kroc Center have access to UHI’s Hallmark programs:
- Group Medical Visits for chronic disease management and patient engagement.
- Care Management following hospitalizations for complex health and social needs.
- Nurse Led Visits: LPN protocols for titration of anti-hypertensive medications and insulin in primary care.
- Addiction Medicine specialty care and integration into UHI primary care.
