Action Needed: Flu Vaccination

December 1, 2017, was the original deadline for all Cooper employees to be in compliance with the Cooper flu vaccination policy. By this date, all employees should have had documentation of having received their flu vaccination for the 2017-2018 flu season or should have signed a declination form stating that they will not be receiving the vaccine. Employees should have the appropriate color sticker for the 2017-2018 flu season on their identification badge reflecting their status.

To date, approximately 90 percent of Cooper employees are compliant with our influenza policy.

For those who have yet to be vaccinated or sign the declination form, you have until Thursday, December 7, 2017, to complete the requirement. Any employee not in compliance by that date will be suspended.

After December 1, you may receive your free flu vaccine from Employee Health at the CCBO, 1 Federal Street, Suite S-200 in Camden by calling 856.382.6432.

If employees have received the vaccine elsewhere they may fax proof of vaccination to Employee Health at 856.735.6496 or scan a copy to EmployeeHealthNurse@cooperhealth.edu.
