Cooper Wellness Incentive Program – Have You Completed the Three Steps?

Cooper offers a Wellness Incentive Program to all employees enrolled in the Cooper Basic, Core, or Standard PPO plans. To be eligible, you must complete all three steps in the program between November 1, 2020 and October 31, 2021:

1. Attest to being a non-tobacco user
2. Receive an annual routine physical
3. Complete the online well-being profile

Upon successful completion, Cooper will waive one bi-weekly medical premium deduction in December.
