Cooper University Health Care’s Population Health team is rapidly growing.  We are seeking enthusiastic, compassionate nurses to transform care delivery by empowering patients for healthier communities.

This new Innovative Delivery program emphasizes a home care Advanced Practice RN team approach that includes an RN and Social Worker. Our Population Health Advanced Practice Nurses practice in collaboration and consultation with other members of the health care team, with a goal to proactively move patients through the system and promote positive patient outcomes. Home visits are required.

Qualified applicants will:

  • Have a Master’s degree in Nursing
  • Have successfully completed the applicable American Nurses Credentialing Center ANCC certification examination for Advanced Nurse Practitioners (APN-C).
  • Have a minimum two years of experience as an Advanced Practice Nurse in Adult Health, Geriatrics, or Primary Care; experience with Home Visits preferred
  • Be able to effectively work with multidisciplinary team, as well as with patients and their families.
  • Be calm and focused in environment of multiple demands, and use good judgment in emergencies.
