Cooper is now hiring a full-time transitions navigator. This individual will provide psycho-social assessments, crisis intervention, and resources referrals to facilitate discharge plans, and/or adjustment to illness, and complex discharge planning for patients and their families. You’ll work with patients, families, and their care team to formulate a discharge plan, coordinate appropriate referrals, and more.
Qualified candidates will have their MSN or RN, BSN with three to five years of health care experience and a current NJ SW license (LCSW preferred) or current NJ RN license.
