We are currently seeking a full-time transitions navigator. In this role, you will provide psychosocial assessments, crisis intervention, and resources referrals, as well as facilitate discharge plans, and/or adjustment to illness, and complex discharge planning for patients and their families. Together with the patient, family, and care team, you’ll formulate a discharge plan based upon a needs assessment.
Qualified candidates have a Master Degree in Social Work (MSW) or a licensed RN, BSN preferred, as well as three to five years of health care experience. A current NJ SW License, LCSW Preferred or current NJ RN License.
