Join us today as a Patient Navigator with our Cancer Genetics Program at MD Anderson Cancer Center at Cooper, you’ll bridge the gap between clinician and patient, providing individual support and assistance in navigating the health care system to provide continuity of care.

In this role, you’ll be working with our current RN Navigator disease team to provide non-clinical support regarding scheduling, pre cert process, follow up calls and record retrieval for timely appointments. This includes calling new patients within 24 hours of visit to CCI to check in regarding follow up appointments and needs, working with CPN, Navigators, Social Work, Dieticians, Call Center, Providers, Radiology scheduling, chemo scheduling, and phone triage to support patients needs, and participating in daily multi disciplinary rounding on inpatient oncology unit to identify needs and opportunities for coordination of care, follow up, and scheduling.

You’ll also act as a member of the Transition of Care team assisting patients in transitioning from inpatient to outpatient areas without interruption in services and care. This includes assisting in connecting patients to services for support (ie: social work, financial counseling), tracking all patient contacts, follow ups, barriers to care, and outcomes, ensuring the delivery of patient care through the coordination of customer services, and participating in interdepartmental and ancillary activities to ensure quality, cost-effective patient care. You’ll be responsible for all core competencies and Cooper standards of care.

Qualified candidates will have a High School Diploma and Clinical training as a Certified MA with BLS, as well as:

  • Experience working in a healthcare office setting, or community outreach, and education.
  • Knowledge of health care, inpatient/outpatient settings, and oncology-specific patient populations.
  • Strong interpersonal skills; including a demonstrated ability to work well with people of various backgrounds, ethnicities, and life experiences.
  • Ability to take direction, follows through with tasks, and work independently.
  • Flexibility, optimism, and a cooperative working style.

Fluency in Spanish preferred.

Competency in basic computer skills and knowledge of using an electronic health records system preferred.
