{"id":109,"date":"2016-04-18T09:40:57","date_gmt":"2016-04-18T13:40:57","guid":{"rendered":"http:\/\/ws05\/blogportal\/enews\/?p=109"},"modified":"2016-04-18T09:40:57","modified_gmt":"2016-04-18T13:40:57","slug":"outpatient-coding-guidelines-uncertain-diagnosis","status":"publish","type":"post","link":"https:\/\/blogs.cooperhealth.org\/ctd\/2016\/04\/outpatient-coding-guidelines-uncertain-diagnosis\/","title":{"rendered":"Outpatient Coding Guidelines: Uncertain Diagnosis"},"content":{"rendered":"<p>Outpatient coding guidelines are used for coding physician services at all\u00a0places of service.\u00a0Outpatient coding\u00a0guidelines are different than those for inpatient facility coding, including acute, short-term, long-term care and psychiatric hospitals.<\/p>\n<p>When coding for physician services a coder may not use a \u201cworking diagnosis.\u201d\u00a0Uncertain diagnoses include the following terms and are documented as:<\/p>\n<p style=\"padding-left: 30px\">\u2022 Probable<br \/>\n\u2022 Suspected<br \/>\n\u2022 Questionable<br \/>\n\u2022 Rule out<br \/>\n\u2022 Likely<br \/>\n\u2022 Terms such as \u201cconsistent with\u201d, \u201ccompatible with\u201d, \u201cindicative of\u201d, \u201csuggestive of\u201d and \u201ccomparable with\u201d also fit the definition of an uncertain diagnosis<\/p>\n<p>The coder is instructed to code the patient\u2019s condition to the highest degree of certainty for that encounter.
