Please take a few minutes to complete the online ballot at <a href=\"http:\/\/bit.ly\/1N74yAe\">http:\/\/bit.ly\/1N74yAe<\/a>\u00a0(This is a private link. Please do not share with anyone who is not a physician living or practicing in NJ).<\/p>\n<p>Use this question as a guide: If a family member was ill, who would you send them to?<\/p>\n<p>Only physicians can participate, and the nominee must be a doctor who lives or works in NJ.<\/p>\n<p>Please be sure to include your practice name, medical license number and signature on your completed ballot.<\/p>\n<p><strong>The deadline for ballot submissions is 5 p.m., Monday, May 2.\u00a0<\/strong><strong>\u00a0<\/strong>You may also print the your ballot and return via fax at 856.234.9912 or mail to SJ Magazine, 1000 Lenola Road, Building Two, Suite 102, Maple Shade, NJ 08052.<\/p>\n<p>&nbsp;<\/p>\n<p>Also, <strong>Best Doctors, Inc.,<\/strong> is now accepting nominations for the 2017-2018 Best Doctors in America\u00ae list.
