As a result, the entire community may be protected, even those who haven’t had the disease or a vaccine. It is believed that herd immunity is achieved when 70% to 90% of the population becomes immune to a particular disease. There are two types of herd immunity – natural immunity caused by infection or immunity achieved through vaccination.

The Zika virus provides an example of natural herd immunity. Some diseases, like Zika which is carried by mosquitos, can sweep through countries and infect a large number of people very quickly. Essentially everyone gets infected, and then becomes immune to the virus, making it impossible for it to continue to spread. This ends the existing virus outbreak. The most recent Zika outbreak started in April 2015, and by November 2016, the World Health Organization announced the end of the epidemic.

Measles provides an example of herd immunity achieved through vaccination. After enough people in the U.S. were vaccinated against the measles, there were very few new measles infections. Once there is herd immunity, if someone with the measles comes into contact with a group of people vaccinated against the measles, the virus has nowhere to spread.

“In theory, natural herd immunity could be a possibility for COVID-19,” says Henry Fraimow, MD, Infectious Disease Specialist at Cooper University Health Care. “However, it is not yet clear that an infection with COVID-19 makes a person immune to a future COVID-19 infection.”

Even if a COVID-19 infection guaranteed long-term immunity, it is estimated that 70% of the U.S. population, or 200 million people, would have to be exposed to and recover from the virus to achieve natural herd immunity. As of late November, about 13.4 million COVID-19 cases had been reported in the U.S., or about 4% of the population, well below the 70% threshold for herd immunity.

“There are several reasons why it’s not a great strategy to let this virus run its course,” says Dr. Fraimow. “Even though the risk of serious disease in younger individuals is lower than in older or immunocompromised individuals, there are still people in all of those age groups who become severely ill and die, and even those who do not die may suffer from long-term complications.”

“Additionally, our society is not always good at separating out people who aren’t at risk from people who are at risk,” Dr. Fraimow continues. “Also, our health care system can easily be overwhelmed during large outbreaks. Finally, we don’t yet fully understand how long immunity lasts. All those factors create some concerns about how herd immunity might work linked to COVID-19.”

To achieve COVID-19 herd immunity and slow the spread of the virus, an effective and safe vaccine may be much more efficient than simply letting the disease run its course. Vaccination, if safe and effective, has the benefit of providing immunity while simultaneously decreasing the number of hospitalizations and deaths. Several vaccine candidates, which have shown promising early results, are now being reviewed by the FDA and may be available soon.

According to the CDC, as of November 24, 2020, large-scale (Phase 3) clinical trials are in progress or are being planned for five COVID-19 vaccines in the U.S. To learn more about the clinical trial process, please click here.

Everyone can help to stop the spread of COVID-19. Remember to wear a face mask when in a public place, maintain good hand hygiene, and practice physical distancing from people who do not live in your household.

Inmunidad Colectiva, Vacunas y COVID-19
