Now, 23 years later, one study indicates that same pill may help men with preexisting heart disease live longer.

According to a study recently conducted in Sweden, Viagra—and medications like it—may reduce the risk of a second or fatal heart attack in men with coronary disease. This disease occurs when fat accumulates along the innermost layer of the coronary arteries. The fatty deposits may develop in childhood and continue to thicken and enlarge throughout life.

“This thickening of the arteries, called atherosclerosis, can decrease or block the flow of blood to the heart,” says Brett Waldman, MD, clinical cardiologist at Cardiac Partners at Cooper and Inspira. “Nearly 16 million Americans suffer from coronary artery disease, and it’s the number one killer of both men and women in the US.”

The decrease in blood flow as a result of atherosclerosis causes a significant increase in the risk of heart attack. ED can be an early warning sign for this disease because the arteries in the penis are so narrow, and proper blood flow is required to make it erect.

“Men often hesitate to see a doctor for ED,” says Dr. Waldman. “However, if men make an appointment to treat ED sooner, it gives us the chance to screen for cardiovascular disease – which can be lifesaving.”

“There are two main treatments for ED,” says Allen D. Seftel, MD, FACS,- Head, Division of Urology. “The first, alprostadil, can be injected into the penis or taken as a suppository. This medication works by dilating the blood vessels so that the penis stiffens.”

“The second treatment option is a class of drugs called PDE5 inhibitors,” continues Dr. Seftel. “These oral medications include Viagra (sildenafil) and Cialis (tadalafil). They work by increasing blood flow to the penis.”

The research from Sweden was published in the Journal of the American College of Cardiology in the spring. The researchers studied 16,500 men who were taking PDE5 inhibitors for ED and approximately 2,000 men who were treated with alprostadil.

All of the participants in the study had stable coronary artery disease. They had not had a heart attack, balloon dilation, or bypass surgery during the six months before they began ED treatments.

“The authors of the study concluded that the men who received the PDE5 inhibitors lived longer and had a lower risk of heart failure, or new heart attack, compared with the men who had alprostadil treatment,” says Dr. Seftel. “Taking more frequent doses of PDE5 inhibitors lowed the risk even further.”

“These benefits may be a result of the medication increasing blood flow, which reduces blood pressure, a leading risk factor for heart disease,” says Dr. Waldman.

Dr. Seftel hypothesizes that the men who were prescribed PDE5 inhibitors were most likely in better cardiovascular shape and were “fit” enough to engage in sexual activity.

That idea agrees with the guidance from both the American Heart Association (AHA) and the American Urological Association (AUA) for physicians when prescribing PDE5 inhibitors to men with preexisting cardiovascular disease. The AHA and AUA created an algorithm for physicians to follow, and generally, the more stable the patient’s heart condition, the safer it is to prescribe these types of drugs.

“This association with Viagra in reducing recurrent cardiac events may be due a to selection bias. Those who were healthy enough to engage in sexual activity were likely in better cardiovascular health, as Dr. Seftel pointed out,” says Dr. Waldman. “It’s important to note that this study included stable patients with known cardiovascular disease and patients should speak to their doctor before taking a PDE5 inhibitor.”

“This was an observational study and the researchers did not prove that the drug caused the health benefits. Further studies are needed to validate the findings,” says Dr. Seftel, “but the news is encouraging, and I await the results of future research on the topic.”

To make an appointment with one a Cooper University Health Care cardiovascular or urologic expert, call 800.8.COOPER (800.826.6737) or visit CooperHealth.org.

Para leer en Español, por favor haga clic aquí In the ’90s, the “little blue pill” was marketed to help men with erectile dysfunction (ED) and Viagra quickly became a household name.
