Or perhaps you just invested in a spin bike and you’re eager to step up your workout game. Regardless of how active you are, make sure your heart is up to the task.

In recent years, several high-profile cases of cardiac arrest—when the heart unexpectedly stops beating—have occurred in people participating in long-distance events, such as marathons and triathlons. These cases are rare, and physicians generally recommend exercise for good heart health as long as you are thoughtful in your approach.

“The type of exercise, intensity level, and many other factors play a role in whether it’s safe for you,” says Mario Caruso, DO, a cardiologist at Cooper University Health Care and member of Cardiac Partners at Cooper and Inspira. “The right exercise program can be different for everyone, so it is important to talk to your doctor before starting anything new.”

Dr. Caruso, a sports cardiologist and competitive endurance athlete, shares some symptoms and risk factors that all active adults should be aware of, whether you are a professional athlete or a weekend warrior.

When to Be Evaluated

If you experience the following symptoms while working out, or immediately afterward, you should seek medical attention:

Chest pain

Fainting or near fainting

Fluttering or pounding heartbeat

Shortness of breath

Most of these symptoms can be addressed if you can schedule a doctor’s visit within a week or two. However, if you lose consciousness during exercise or if you have chest pain that lingers after you stop activity, you should go to the nearest emergency room. The cause may be as simple as skipping breakfast and having low blood sugar, but there’s also a chance that these symptoms could be a sign of something serious.

Risk Factors

People with underlying heart disease are at increased risk for cardiac arrest when exercising, so they should talk with their doctor about the best way to incorporate activity into their lives.

“We want our high-risk patients to be particularly careful about taking on any activities that nudge them toward their maximum heart rate, such as sprinting,” Dr. Caruso says. “Doing any type of activity for a long period of time—like long-distance running—can also be a problem because it can lead to dehydration and electrolyte imbalances, which are hard on the heart. That can tip the scale and lead to a sudden cardiac arrest.”

Those who have a heart condition should also avoid caffeine, drug, and alcohol use.

However, if you are younger than 35 and don’t have an underlying genetic heart condition, your chances of having a sudden cardiac arrest are slim.

“Most of the time, if somebody older than 35 has a sudden cardiac arrest, it’s from something like heart vessel blockages rather than a problem with the structure of the heart that was there from birth,” Dr. Caruso says.

The Exercise Sweet Spot

For people who are not at high risk for cardiac disease, the American College of Cardiology recommends 150 to 300 minutes per week of moderate intensity exercise or 75 to 150 minutes a week of vigorous exercise to maintain adequate cardiovascular fitness. Depending on a variety of factors, people who are at high risk still may benefit from low-to-moderate intensity exercise, which helps them to avoid other diseases, such as diabetes, heart disease, and stroke.

“There’s a fine balance we need to strike with people who are at higher risk for cardiac arrest,” Dr. Caruso says. “But with heart monitors, stress testing, and ultrasounds, we can calculate a risk score to figure out how long and how vigorously you can exercise. Everyone is different, but we have the tools to figure out what is right for you. We want to help you stay as active as possible.”

Dr. Caruso sees patients in our Turnersville and Voorhees offices.
