A new law in New Jersey now requires hospitals and clinical laboratories to provide you with a specific opportunity to include additional information in your medical record on race, ethnicity, sexual orientation, and gender identity. The information you provide will be included in your secure personal medical record that can be used by your Cooper provider in your treatment.<\/p>\n

To update your medical record and include additional information on race, ethnicity, sexual orientation, and gender identity log in to your myCooper account and navigate to the Personal Information Section.<\/p>\n

Click the Edit <\/strong>button (circled below) and you will be able to update each category. You will be able to add information, if you wish, or click Choose not to disclose<\/strong> as shown below.<\/p>\n

When you hit the Edit<\/strong> button, you will be able to click each category to include information or hit the Choose not to disclose<\/strong> answer.\u00a0 Once updated, hit the Save Changes<\/strong> button and your information will be updated.<\/p>\n
