From abdominal pain and bloating to chronic constipation and bleeding, these symptoms can be debilitating and severely impact a person’s quality of life.

GI symptoms often can be mistaken for other issues, such as indigestion or a stomach bug. When symptoms persist, however, it’s important to seek medical attention.

Irritable bowel syndrome (IBS) and inflammatory bowel disease (IBD) are conditions that affect the gastrointestinal tract, but they are separate disorders with different characteristics, causes, and treatment plans.

Irritable Bowel Syndrome (IBS)

Irritable bowel syndrome (IBS) is a common condition characterized by abdominal discomfort and associated with altered bowel movements.

The exact cause of IBS is unknown, but it is thought to involve a combination of factors such as abnormal intestinal contractions, heightened sensitivity to pain, changes in gut bacteria, and increased stress response.

Symptoms of IBS vary and affect some people more severely than others.

Common symptoms of IBS include:

Stomach gas, bloating, and swelling

Abdominal pain that may be relieved by a bowel movement

Altered bowel habits, either diarrhea or constipation, and frequently both

“Treatment for IBS usually involves lifestyle modifications,” says Yize Richard Wang, MD, PhD, AGAF, gastroenterologist, Associate Director, Inflammatory Bowel Disease Center, at Cooper University Health Care and Associate Professor of Medicine at Cooper Medical School of Rowan University. “Our team works closely with patients to create a treatment plan that can include dietary changes, stress management techniques, and, in some cases, medications to alleviate specific symptoms.”

If your symptoms have lasted for more than six to eight weeks, or if you are experiencing persistent pain, bleeding, or unexplained weight loss, this may indicate a more serious condition, such as Crohn’s disease, ulcerative colitis, or colorectal cancer.

Inflammatory Bowel Disease (IBD): Crohn’s vs. Ulcerative Colitis, Similarities and Differences

Inflammatory bowel disease (IBD) is classified as a disease, rather than a group of symptoms, like IBS. IBD is a term for two conditions: Crohn’s disease and ulcerative colitis (UC). Both conditions involve chronic inflammation of the digestive tract and there are multiple ways to manage these diseases to keep them in remission (without active symptoms).

In some severe cases of UC, your doctor may recommend a proctocolectomy, a curative surgery to remove the entire colon, including the rectum. Proctocolectomy, however, is not commonly recommended for people with Crohn’s disease.

Ulcerative colitis and Crohn’s disease often develop in teenagers and young adults, although they can occur in people of any age. Both diseases affect men and women equally, and those with a family history of IBD are at higher risk. For an accurate diagnosis, your doctor will use a combination of lab tests and endoscopic procedures.

Both Crohn’s disease and UC can have periods of remission and flare-ups, where symptoms may worsen.

Symptoms of IBD

“Crohn’s disease and UC have many similar symptoms,” Dr. Wang says. “The most common are blood in stool and the urge to use the bathroom frequently throughout the day, sometimes at night.”

Other common symptoms include:

Abdominal pain

Diarrhea

Weight loss

Fatigue

Many people with IBD experience mild to moderate symptoms and some have long periods of remission. However, you should see your health care provider if you have ongoing diarrhea that doesn’t respond to medications, diarrhea that wakes you from sleep, or an unexplained fever lasting more than two days.

Key Differences

Although Crohn’s disease and UC share similarities, they have distinct differences.

Ulcerative colitis is limited to the large intestine (the colon). Crohn’s disease can occur anywhere throughout digestive tract, from “the gum to the bum” or the mouth to the anus.

Ulcerative colitis usually involves continuous areas of inflammation starting from the rectum, whereas Crohn’s disease can have a skip pattern, with normal parts of the intestine between affected areas.

Ulcerative colitis primarily affects the inner lining of the colon. Crohn’s disease can affect all layers of the intestinal wall, causing stricture (narrowing) and fistula (abnormal connection to an adjacent organ).

With UC, the pain is typically in the left side of the abdomen. With Crohn’s disease, pain can occur in the mid or right abdomen.

“One of the most important things about gastrointestinal disorders, specifically IBD, is early detection,” Dr. Wang says. “Risks of not treating IBD are higher frequency of flare-ups, bowel obstructions, irreversible bowel damage, and increased risk of colon cancer.”

Trust the Digestive Health Experts at Cooper

The award-winning Digestive Health Institute at Cooper provides superior care with nationally recognized experts.
