When water is trapped in the ear, it creates a moist environment that fosters bacterial and fungal growth.

“OE can happen any time of year, but we generally see a spike in the summer months as people are spending more time enjoying water activities.” says Donald Solomon, MD, an otolaryngologist (ear, nose, and throat specialist) and Director of Pediatric Otolaryngology and Pediatric Otology at Cooper University Health Care. “Swimmer’s ear can occur in both adults and children, but it is more commonly seen in children. Another risk factor is earwax build-up, which can trap water when your head is submerged.”

Children are more prone to OE because they often spend more time splashing in water while swimming and bathing. They may also be less vigilant about drying their ears properly after water exposure.

Adults who engage in water activities, such as swimming, surfing, or going to a sauna, are also at risk for swimmer’s ear. People who have excessive earwax production or eczema are at greater risk. Likewise, those who use devices like hearing aids, headphones, ear plugs, or earbuds regularly may be more susceptible to an infection, as these devices often push wax deep into the ear canals.

Signs and Symptoms of Swimmer’s Ear

The signs and symptoms of swimmer’s ear can vary in severity, but often include:

Ear pain: This can range from mild discomfort to severe, sharp, or throbbing pain. The pain may worsen when you tug on the ear, push on the small flap of cartilage just in front of your ear canal (called the tragus), or move your jaw.

Itching: The affected ear may feel itchy inside.

Drainage: Swimmer’s ear can cause the ear to produce discharge. The discharge may be clear, pus-like, or yellowish. It can also have an unpleasant odor.

Redness and swelling: As swimmer’s ear progresses, the outer ear may appear red, swollen, or inflamed. The ear canal can swell shut if a swimmer’s ear infection progresses without treatment.

Decreased hearing: Inflammation and fluid buildup can impair hearing in the affected ear. Sounds may be muffled or distorted.

“If you think you or your child may have swimmer’s ear, see your doctor right away,” Dr. Solomon says. “These infections are usually treated with antibiotic ear drops and, if advanced, an oral antibiotic may be added. Your physician will also want to rule out any other ear issues.”

Tips to Prevent Swimmer’s Ear

Dr. Solomon offers the following tips to help prevent OE.

Keep ears dry. After swimming or bathing, gently dry your ears with a towel or use a hair dryer on low heat for a few minutes to dry out the ears. Hold the dryer at arm’s length as to not to cause burns, especially to children. After swimming, you can also lean forward and tilt your head to the side to allow any trapped water to drain out of the ear canal. If you go to a sauna, you should make sure you dry your ears thoroughly after, or even consider bringing a cotton ball to keep the inside of your ear dry while you are there.

Consider protecting your ears if you have a history of swimmer’s ear. Use earplugs (Doc’s Pro Plugs), the Ear Bandit head wrap, or a swimming cap to keep water out of your ears while swimming or engaging in water activities. Check with your health care provider about using ear-drying drops before or after swimming.

Avoid inserting objects. Refrain from inserting cotton swabs, fingers, or any other objects into your ear canal, especially deeply in the ear canal as these can push debris or wax further in trapping it. If you think the ear canal could be blocked by ear wax, check with your health care provider.

“Prevention is the best way to combat swimmer’s ear,” Dr. Solomon says. “By keeping these tips in mind before and after swimming, you can greatly reduce the risk of ear infection.”

ENT Experts at Cooper

Otolaryngologists can diagnose and treat a variety of ear, nose, and throat conditions, including tinnitus, vertigo, nasal polyps, swallowing and vocal cord issues, sleep conditions, and tumors of the head and neck.
