{"id":10721,"date":"2019-12-09T15:18:10","date_gmt":"2019-12-09T20:18:10","guid":{"rendered":"http:\/\/blogs.cooperhealth.org\/weeklyrounds\/?p=10721"},"modified":"2019-12-10T13:55:42","modified_gmt":"2019-12-10T18:55:42","slug":"flu-vaccination-deadline-december-11-2019","status":"publish","type":"post","link":"https:\/\/blogs.cooperhealth.org\/weeklyrounds\/2019\/12\/flu-vaccination-deadline-december-11-2019\/","title":{"rendered":"Flu Vaccination Deadline: December 11, 2019"},"content":{"rendered":"<p>All employees must adhere to the flu vaccination policy by <strong>Wednesday,<\/strong> <strong>December 11, 2019<\/strong>.<\/p>\n<p>The <a href=\"https:\/\/cooperhealth.policytech.com\/dotNet\/documents\/?docid=10613\"><strong>full policy<\/strong><\/a>, along with a <a href=\"http:\/\/blogs.cooperhealth.org\/weeklyrounds\/files\/2019\/09\/Cooper-Flu-Immunization-Schedule-2019-2020-2.pdf\"><strong>schedule<\/strong><\/a> of vaccination dates, is available on the Pulse. <strong>Please note: <\/strong>You may receive your flu vaccination at any of the times and locations listed on the schedule.<\/p>\n<p>If you receive your vaccination at a private physician office or a public clinic, you must submit proof of immunization to Employee Health <strong>as soon as possible <\/strong>to receive a blue sticker. Please email your proof of vaccination to <a href=\"mailto:McGinty-Reynolds-Den@cooperhealth.edu\">Denise McGinty Reynolds<\/a> or <a href=\"mailto:Peacock-Kimberly@cooperhealth.edu\">Kimberly Peacock<\/a>, or submit it to Employee Health via fax at 856.735.6496.<\/p>\n<p><strong>Highlights of the vaccination policy include:<\/strong><\/p>\n<ul>\n<li>All Cooper employees are required to get an annual flu vaccination or sign a declination statement.<\/li>\n<li><strong>Blue<\/strong> ID badge stickers will be given to employees who receive the flu vaccination.\u00a0Yellow ID badge stickers will be given to employees who sign a declination statement.<\/li>\n<li>Employees who opt out of vaccination are required to wear a surgical mask while in clinical areas to prevent the spread of the virus.<\/li>\n<li>Employees who are on a leave of absence will receive a letter at home with instructions to either visit Employee Health for a flu vaccination before returning to work, or provide proof that they received their flu vaccine elsewhere.<\/li>\n<li>Employees and managers are able to\u00a0view and manage flu compliance in Lawson through Employee and Manager Self Service. Additionally, managers will be able to view the flu vaccine compliance status for their entire staff.<\/li>\n<\/ul>\n<p><a href=\"http:\/\/blogs.cooperhealth.org\/weeklyrounds\/files\/2017\/11\/EmpEEHealthTipSheet_v10-Edited.pdf\"><strong>Download employee instructions<\/strong><\/a><strong>.<\/strong><\/p>\n<p><strong><a href=\"http:\/\/pulse.cooperhealth.edu\/wp-content\/uploads\/2018\/09\/Flu-Manager-instructions.pdf\">Download manager instructions<\/a>.<\/strong><\/p>\n<p>This year, Cooper is administering the quadrivalent flu vaccine.
