Each quarter, Population Health identifies an employee who exemplifies its values \u2013 Innovation, Patient Empowerment, Commitment to Excellence, Compassionate Care.<\/p>\n<p>Last quarter, Katrina Donato,\u00a0Care Coordinator,\u00a0was recognized for her innovation when working with a patient after a hospital admission. All employees are encouraged to nominate someone who drives Population Health outcomes.
