The award honors a person who improves population health by enhancing patient outcomes, improving quality metrics, lowering costs, and increasing satisfaction. Nominees do not need to be part of the Population Health team \u2013 they can work in any department, as long as they demonstrate and influence the Population Health core values. Congratulations to our previous winner, Rachel Adams, APN, Urban Health Institute, for her impact on patient care.
