The state law eliminates previously allowed religious and personal exemptions, but does allow for limited medical exemptions.\u00a0If you are seeking a medical exemption, please fill out the medical exemption form <a href=\"https:\/\/request.cooperhealth.org\/apps\/wp-content\/uploads\/2022\/09\/njstatemedicalexemption_LOGO.pdf\"><strong>here<\/strong><\/a> and return it to Employee Health at <strong>flu@Cooperhealth.edu<\/strong> by <strong>November 1, 2022<\/strong>. The form will <strong>only<\/strong> be processed if you return it to Employee Health \u2013 <strong>please do not submit it to your manager.<\/strong><\/p>\n<p>It remains unclear whether this law applies to those who work for a health system, but do not work in the hospital or another Department of Health licensed facility.\u00a0Therefore,\u00a0<b>if you are seeking a religious or personal exemption from the flu vaccination, and you do not work in the hospital or a licensed facility,<\/b> please fill out this <a href=\"https:\/\/update.cooperhealth.org\/non-medical-exemption-request-flu\/\">form here<\/a><b>\u00a0<\/b>by\u00a0<b>November 1, 2022<\/b>. You will be advised of next steps.<\/p>\n<p><strong>Additional information:<\/strong><\/p>\n<p><strong>All Cooper team members, physicians, residents, students,\u00a0and volunteers <\/strong>may receive the flu vaccination from an Employee Health nurse at any of the times and locations listed on the schedule. Additionally, team members who would like to receive a COVID-19 booster shot may do so at any of the flu clinic locations. Walk-ins are welcome and no appointment is necessary.<\/p>\n<ul>\n<li>This year, Cooper will administer the quadrivalent flu vaccine, which is designed to protect against four different strains of flu virus, including two influenza A viruses and two influenza B viruses.<\/li>\n<li>Team members who receive the vaccination at a private physician office or a public clinic must submit proof of immunization via email to Employee Health at <strong>flu@Cooperhealth.edu<\/strong>. This information will <strong>only<\/strong> be processed if you return it to Employee Health \u2013 <strong>please do not submit it to your manager. <\/strong>All proof of vaccination must have:<\/li>\n<\/ul>\n<ul>\n<li>Your name.<\/li>\n<li>Your birthdate.<\/li>\n<li>The date the vaccine was administered.<\/li>\n<li>The lot number of the vaccine.<\/li>\n<li>The expiration date.<\/li>\n<\/ul>\n<ul>\n<li>Team members who are on a leave of absence will receive a letter at home with instructions on how to receive their flu vaccination.<\/li>\n<li>Cooper team members can view their flu vaccination status in Lawson Self Service. For Lawson instructions, <a href=\"http:\/\/blogs.cooperhealth.org\/weeklyrounds\/files\/2017\/11\/EmpEEHealthTipSheet_v10-Edited.pdf\"><strong>click here<\/strong><\/a>.
