The Population Health team is composed of both clinical and non-clinical staff including APNs, RNs, health advisors, and community health screeners who provide care coordination to patients.<\/p>\n<p>Cooper experts provided education on a variety of topics.<\/p>\n<ul>\n<li>Rebecca Bryan, DNP, AGPCNP, APN, Director of Community Engagement and Professional Development, Rutgers University School of Nursing-Camden, discussed how to identify the signs and symptoms of depression in patients, and best practices and resources that can improve quality of life.<\/li>\n<li>Cooper cardiologist Fredric Ginsberg, MD, discussed congestive heart failure, the principles of heart failure management, epidemiology, medical therapy, and other aspects of the disease.<\/li>\n<li>Cooper endocrinologist Gregory Barone, DO, discussed diabetes management.<\/li>\n<li>Cooper dietician and diabetes educator, Jaclyn Cimiluca, RD, discussed understanding the relationship between food and culture as well as distinguishing cultural food practices in various ethnic and racial groups.<\/li>\n<li>Eileen Craig and Sherry Mazer, from Cooper\u2019s Compliance department, discussed proper documentation in Epic, including the purpose of documentation, a framework for documentation, and the do\u2019s and don\u2019ts of documentation.<\/li>\n<li>Population Health Director Stephanie Tonetta, APN, provided a recap of the Web of Population Health and Care Coordination.<\/li>\n<\/ul>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-16963\" src=\"https:\/\/blogs.cooperhealth.org\/weeklyrounds\/files\/2022\/12\/2022_Pop_health_symposium_1671562500.png\" alt=\"\" width=\"519\" height=\"421\" srcset=\"https:\/\/blogs.cooperhealth.org\/weeklyrounds\/files\/2022\/12\/2022_Pop_health_symposium_1671562500.png 519w, https:\/\/blogs.cooperhealth.org\/weeklyrounds\/files\/2022\/12\/2022_Pop_health_symposium_1671562500-300x243.png 300w\" sizes=\"auto, (max-width: 519px) 100vw, 519px\" \/><\/p>\n","protected":false},"excerpt":{"rendered":"<p>Cooper\u2019s Population Health team recently participated in a symposium that provided education on different chronic diseases and the best management approach for patients.
