A \u201cGood Catch\u201d is a potential error that\u00a0never<\/u>\u00a0reaches the patient because of identification by a\u00a0team\u00a0member. These events can also be referred to as a near miss, and they are often the key to identifying system and process issues before errors have a chance to harm a patient, visitor, or team member.<\/p>\n

This is why Cooper has instituted a Good Catch program. The program encourages the reporting of Good Catches via the EARS reporting system. These events are reviewed for opportunities for improvement and are also nominated for a quarterly Good Catch award. We believe that our team members and providers deserve to be recognized for all of the hard work that they put in each and every day, especially when they help to identify key opportunities for improvement.<\/p>\n

Below are the nominees for this quarter\u2019s Good Catch.<\/p>\n

Nicole Young, MRI Tech<\/strong>. Paperwork indicated no pacemaker. Patient disoriented. Tech evaluated and felt a pacemaker. Confirmed by x-ray. Injury avoided. Click here<\/a> to watch Nicole tell her story in her own words.<\/p>\n

Nancy Gibson, RN<\/strong>. Infant with trisomy 21 for discharge. On review, a stat lab was not done. Test done and resulted abnormal. Baby transferred to NICU.<\/p>\n
