The deadline to receive the vaccination or submit an exemption form is <strong>Thursday, December 7, 2023<\/strong>.\u00a0<a href=\"https:\/\/blogs.cooperhealth.org\/weeklyrounds\/files\/2023\/10\/Flu-Vaccine-Schedule-2023-2024_updated-10-12-23.pdf\"><strong>Click here<\/strong><\/a>\u00a0for the schedule.<\/p>\n<p>New Jersey law mandates individuals who work in a hospital, nursing home, or home health care agency must receive an influenza vaccination. The state law allows for limited medical exemptions.\u00a0If you are seeking a medical or religious exemption, please fill out the medical exemption form\u00a0<a href=\"https:\/\/request.cooperhealth.org\/apps\/wp-content\/uploads\/2022\/09\/njstatemedicalexemption_LOGO.pdf\"><strong>here<\/strong><\/a>\u00a0and return it to Employee Health at\u00a0<a href=\"mailto:flu@cooperhealth.edu\"><strong>flu@cooperhealth.edu<\/strong><\/a>\u00a0by\u00a0<strong>Thursday,<\/strong>\u00a0<strong>December 7, 2023<\/strong>. The form will\u00a0<strong>only<\/strong>\u00a0be processed if you return it to Employee Health \u2013\u00a0<strong>please do not submit it to your manager.<\/strong><\/p>\n<p>If you are seeking a personal exemption from the flu vaccination, and you do not work in the hospital or a licensed facility, please fill out\u00a0<a href=\"https:\/\/update.cooperhealth.org\/non-medical-exemption-request-flu\/?_gl=1*abkgpa*_ga*MTEyMDk1MTg1MS4xNjk1MjI0MjIz*_ga_Q7FC98J25G*MTY5Njg4NjQ4Mi4xMi4xLjE2OTY4ODY0ODcuMC4wLjA.\/\/\"><strong>this form<\/strong><\/a>\u00a0and return it to Employee Health at\u00a0<a href=\"mailto:flu@cooperhealth.edu\"><strong>flu@cooperhealth.edu<\/strong><\/a>\u00a0by\u00a0<strong>Thursday,<\/strong>\u00a0<strong>December 7, 2023<\/strong>. The form will\u00a0<strong>only<\/strong>\u00a0be processed if you return it to Employee Health \u2013\u00a0<strong>please do not submit it to your manager.<\/strong><\/p>\n<p>Once flu season is officially declared, team members\u00a0<strong><u>who have received an approved exemption<\/u>\u00a0<\/strong>and have not received a flu vaccination must wear a mask in all Cooper facilities until flu season has ended.<\/p>\n<p><strong>Additional information:<\/strong><\/p>\n<p>All Cooper team members, physicians, residents, students,\u00a0and volunteers may receive the flu vaccination from an Employee Health nurse at any of the times and locations listed on the\u00a0<a href=\"https:\/\/blogs.cooperhealth.org\/weeklyrounds\/files\/2023\/10\/Flu-Vaccine-Schedule-2023-2024_updated-10-12-23.pdf\"><strong>schedule<\/strong><\/a><strong>.<\/strong><\/p>\n<ul>\n<li>This year, Cooper will administer the quadrivalent flu vaccine, which is designed to protect against four different strains of flu virus, including two influenza A viruses and two influenza B viruses.<\/li>\n<li>Team members who receive the vaccination at a private physician office or a public clinic must submit proof of immunization via email to Employee Health at\u00a0<a href=\"mailto:flu@cooperhealth.edu\"><strong>flu@cooperhealth.edu<\/strong><\/a>. This information will\u00a0<strong>only<\/strong>be processed if you return it to Employee Health \u2013\u00a0<strong>please do not submit it to your manager.\u00a0<\/strong>All proof of vaccination must have:\n<ul>\n<li>Your name.<\/li>\n<li>Your birth date.<\/li>\n<li>The date the vaccine was administered.<\/li>\n<li>Manufacturer of vaccine.<\/li>\n<li>The lot number of the vaccine.<\/li>\n<li>The expiration date.<\/li>\n<li>The location of the injection site: right arm or left arm.<\/li>\n<\/ul>\n<\/li>\n<li>Team members who are on a leave of absence will receive a letter at home with instructions on how to receive their flu vaccination.<\/li>\n<li>Team members can view their flu vaccination status in iCooper under \u201cMy Health.\u201d<\/li>\n<li><strong>Managers<\/strong>: Please be sure your team member roster is current.<\/li>\n<\/ul>\n<p>If you have questions, please email\u00a0<a href=\"mailto:flu@cooperhealth.edu\"><strong>flu@cooperhealth.edu<\/strong><\/a><strong>.<\/strong><\/p>\n","protected":false},"excerpt":{"rendered":"<p>The 2023 Flu Vaccination Program is ending soon.
