If you are enrolled in Cooper\u2019s health plan, you should have received new Horizon identification cards last month.<\/p>\n<p>The new Horizon cards only have the name of the subscriber printed on the card, which is different from our previous plan. Therefore, only your name will be on your spouse or dependents\u2019 card.
