Congratulations to Rae Easterday, Manager of Voorhees Internal Medicine, for being selected as the Q1 Population Health Quarterly Award Winner. Rae has been instrumental in improving population health outcomes by putting initiatives in place to increase annual wellness visits and training team members to help reach and exceed our goals.\u00a0Congrats to Rae on a job well done and for your impact on population health core values and patient outcomes.<\/p>\n

Q2 award nominations are now open. We welcome submissions from all departments and teams within the organization. To nominate someone, please complete a short webform here<\/a> by Friday, July 5, 2024. The winner will be announced at an upcoming Population Health Department meeting.<\/p>\n
