All reports are confidential and may be entered anonymously. Log on to EARS with your Cooper network username and password.<\/p>\n<p><strong>Below is a list of reportable events:<\/strong><br \/>\n\u2022 Adverse Drug Reaction<br \/>\n\u2022 Airway Management<br \/>\n\u2022 Blood Product<br \/>\n\u2022 Diagnosis\/Treatment<br \/>\n\u2022 Diagnostic Imaging<br \/>\n\u2022 Equipment\/Medical Device<br \/>\n\u2022 Fall<br \/>\n\u2022 Infection<br \/>\n\u2022 IV\/Vascular Access Device<br \/>\n\u2022 Lab\/Specimen<br \/>\n\u2022 Maternal\/Childhood<br \/>\n\u2022 Medication\/Fluid<br \/>\n\u2022 Professional Conduct<br \/>\n\u2022 Safety\/Security<br \/>\n\u2022 Skin Tissue<br \/>\n\u2022 Surgery\/Procedure<br \/>\n\u2022 Tube\/Drain<\/p>\n<p><a href=\"http:\/\/blogs.cooperhealth.org\/weeklyrounds\/files\/2015\/08\/weeklyrounds.jpg\" rel=\"lightbox[2218]\"><img loading=\"lazy\" decoding=\"async\" class=\"  wp-image-2221 alignnone\" src=\"http:\/\/blogs.cooperhealth.org\/weeklyrounds\/files\/2015\/08\/weeklyrounds.jpg\" alt=\"weeklyrounds\" width=\"304\" height=\"171\" \/><\/a><\/p>\n<p>Contact Patient Relations at 856.342.2432 to report any complaints or grievances.
