Please <a href=\"https:\/\/blogs.cooperhealth.org\/weeklyrounds\/files\/2024\/10\/Flu-Vaccine-Schedule-2024-2025-FLYER_10-14-24.pdf\">click here<\/a> for the updated schedule.<\/p>\n<p>The 2024 Flu Vaccination Program will conclude on\u00a0<strong>Monday, December 9, 2024<\/strong>. Cooper team members are able to receive COVID-19 vaccines for free at any of Cooper\u2019s flu vaccination clinics.<\/p>\n<p>An annual flu vaccination is mandated by\u00a0New Jersey state law for individuals who work in a hospital, nursing home, or home health care agency.<\/p>\n<p>State law allows for limited medical exemptions.\u00a0If you are seeking a medical exemption, please fill out the medical exemption form\u00a0<a href=\"https:\/\/blogs.cooperhealth.org\/weeklyrounds\/files\/2024\/08\/NJ-State-Medical-Exemption-Form.pdf\"><strong>here<\/strong><\/a>\u00a0and return it to Employee Health at\u00a0<a href=\"mailto:flu@cooperhealth.edu\"><strong>flu@cooperhealth.edu<\/strong><\/a>\u00a0by\u00a0<strong>Friday,<\/strong>\u00a0<strong>November 15, 2024<\/strong>. The form will\u00a0<strong>only<\/strong>\u00a0be processed if you return it to Employee Health.<\/p>\n<p>If you are seeking a personal exemption from the flu vaccination, and you do not work in the hospital or a licensed facility, please fill out\u00a0<a href=\"https:\/\/blogs.cooperhealth.org\/weeklyrounds\/files\/2024\/08\/Non-Medical-Exemption-Form-2024.pdf\"><strong>this form<\/strong><\/a>\u00a0and return it to Employee Health at\u00a0<a href=\"mailto:flu@cooperhealth.edu\"><strong>flu@cooperhealth.edu<\/strong><\/a>\u00a0by\u00a0<strong>Friday,<\/strong>\u00a0<strong>November 15, 2024<\/strong>. The form will\u00a0<strong>only<\/strong>\u00a0be processed if you return it to Employee Health.<\/p>\n<p>Once flu season is officially declared, team members\u00a0<u>who have received an approved exemption<\/u>\u00a0and have not received a flu vaccination must wear a mask in all Cooper facilities until flu season has ended.<\/p>\n<p><strong>Additional information:<\/strong><\/p>\n<p>All Cooper team members, regardless of role may receive the flu vaccination from an Employee Health nurse at any of the times and locations listed on the\u00a0<a href=\"https:\/\/blogs.cooperhealth.org\/weeklyrounds\/files\/2024\/10\/Flu-Vaccine-Schedule-2024-2025-FLYER_10-14-24.pdf\"><strong>schedule<\/strong><\/a>.<\/p>\n<ul>\n<li>This year, Cooper will administer the trivalent flu vaccine, which is designed to protect against three different strains of the flu virus.<\/li>\n<li>Team members who receive the vaccination at a non-Cooper physician office, a pharmacy, or a public clinic must submit proof of immunization via email to Employee Health at\u00a0<a href=\"mailto:flu@cooperhealth.edu\"><strong>flu@cooperhealth.edu<\/strong><\/a>. This information will\u00a0<strong>only<\/strong>\u00a0be processed if you return it to Employee Health. All proof of vaccination must have:\n<ul>\n<li>Your name.<\/li>\n<li>Your birth date.<\/li>\n<li>The date the vaccine was administered.<\/li>\n<li>Manufacturer of vaccine.<\/li>\n<li>The lot number of the vaccine.<\/li>\n<li>The expiration date.<\/li>\n<li>The location of the injection-site: right arm or left arm.<\/li>\n<\/ul>\n<\/li>\n<li>Team members who receive their flu vaccine in a Cooper physician office do\u00a0<strong>NOT<\/strong>\u00a0need to email proof to Employee Health.\u00a0<strong>The flu vaccine information will automatically load from Epic into iCooper.<\/strong><\/li>\n<li>Team members who are on a leave of absence will receive a letter at home with instructions on how to receive their flu vaccination.<\/li>\n<li>Cooper team members can view their flu vaccination status in iCooper under \u201cMy Health.\u201d For instructions, please refer to the\u00a0<a href=\"https:\/\/icooper.cooperhealth.org\/team-member-library\/\"><strong>iCooper library<\/strong><\/a>.<\/li>\n<\/ul>\n<p>If you have any questions, please email\u00a0<a href=\"mailto:flu@cooperhealth.edu\"><strong>flu@cooperhealth.edu<\/strong><\/a>.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Please note that the following information does not apply to Cooper at Cape team members, as their program has already concluded.
