Your Cooper benefits can support you in achieving your wellness goals.<\/p>\n<p>As a reminder, team members enrolled in Cooper\u2019s Horizon Blue Cross Blue Shield of New Jersey\u2019s health plan can earn <strong>up to $240 per year<\/strong> in wellness incentives by tracking their workouts with the<strong> Horizon<em>b<\/em>Fit<\/strong> program. All members enrolled in the medical plan age 18 and older \u2013 including your dependents \u2013 enrolled in the medical plan age 18 and older are eligible to participate in the fitness incentive program. For more information, <a href=\"https:\/\/blogs.cooperhealth.org\/weeklyrounds\/files\/2025\/01\/2025_Cooper_Physical_Well_Being_Program.pdf\"><strong>click here<\/strong><\/a>.<\/p>\n<p><strong>E<\/strong><strong>nrolled in Horizon<em>b<\/em>Fit, <\/strong><strong>and<\/strong><strong> earn a $20 reward for each month that you:<\/strong><\/p>\n<ul>\n<li>Work out at home 12 or more days a month, and record and submit your workout using the Fit-At-Home<sup>SM<\/sup>\u00a0feature, or<\/li>\n<li>Walk 10,000 steps a day for at least 12 days a month, or<\/li>\n<li>Visit one of 4,000 participating fitness facilities 12 or more days a month, or<\/li>\n<li>Complete any combination of visiting a participating facility, working out at home, or walking 10,000 steps for a total of at least 12 days a month.<\/li>\n<\/ul>\n<p><strong>How to Enroll<\/strong><\/p>\n<p>Enrolling is free and easy at\u00a0<a href=\"https:\/\/members.advantahealth.com\/horizonbfitmobile\"><strong>Horizon<em>b<\/em>Fit.com<\/strong><\/a>. You can verify your eligibility using your member ID number to set up your account. If your fitness facility does not participate with Horizon<em>b<\/em>Fit, you can nominate it for inclusion.
