This recognizes innovative strategies that improve clinical health outcomes, enhance care coordination, and advance public health policy to create sustainable impact.<\/li>\n<li><a href=\"https:\/\/www.cooperhealth.org\/services\/cooper-idd-patient-and-family-support-program\"><strong>Intellectual\/Developmental Disabilities (I\/DD) Program<\/strong><\/a> \u2013 finalist in the <em>Advancing Health Equity<\/em> category. This celebrates models and strategies that reduce health disparities, address social and economic barriers, and ensure equitable access to care\u2014especially for special populations, including older adults and individuals with intellectual disabilities, autism, behavioral health challenges, or physical disabilities.<\/li>\n<\/ul>\n<p>As finalists, they\u2019ve entered the voting round, and your support is crucial.
