If you have not enrolled or re-enrolled, you must take action!<\/p>\n<p>All benefits-eligible employees are required to re-enroll in the medical plan or waive coverage in 2016, and also attest to being a non-tobacco or tobacco user.<\/p>\n<p>If you do not enroll or waive medical coverage during Open Enrollment, you will be defaulted in the Cooper Core Plan \u2013 Employee Only coverage and will be charged the full rate for tobacco users effective January 1, 2016.<\/p>\n<p><a href=\"http:\/\/www.cooperhealth.org\/sites\/cooper\/files\/site\/Open%20Enrollment%20FAQ.pdf\">Click here for open enrollment FAQs<\/a>.<\/p>\n<p>This year, your confirmation statement will not be mailed to your home.
