\"group\"The Patient Safety Department would like to congratulate the Good Catch winners for Q2: Pharmacy Technicians Angela Marino and Julia Mosley.<\/p>\n

During Angela and Julia\u2019s medication reconciliation process they worked together to identify three different patient charts combined into one. Identifying this combination of charts and facilitating correction ensured that we have the correct medication information with the correct patient, therefore improving patient safety. This Good Catch impacts two of The Joint Commission\u2019s National Patient Safety Goals: Improving the accuracy of patient identification and medication reconciliation.<\/p>\n

Kudos to both Angela and Julia for their continued dedication and excellence regarding Patient Safety!<\/p>\n
