To be eligible, you must complete all three steps, listed below, between <strong>November 1, 2016, and October 31, 2017<\/strong>. Upon successful completion, you will not be charged for one bi-weekly Medical Plan payroll deduction in December 2017.<\/p>\n<p>Step 1. Complete the annual physical.<\/p>\n<p>Step 2. Attest to non-tobacco use during 2017.<\/p>\n<p>Step 3.
