The Cooper Population Health team presented a program overview of Cooper Population Health initiatives at the Burlington County Coalition meeting on December 6, 2018. Cooper Population Health is staffed with APNs, care coordinators, and health coaches who help coordinate care for patients with complex and chronic medical conditions. The ultimate goals of the team are to facilitate patient self-care management through patient engagement; identify social barriers to care; and achieve a reduction in patient hospitalizations, re-admissions, and health care costs.<\/p>\n

The presentation included 2017 results which show a program cost savings of $1.2 million.<\/p>\n
