Meet Cooper’s Care Management Team

Cooper’s Care Management Team provides a wide variety of experiences and expertise to our primary care practices in an effort to support patients with chronic conditions.
Between seven PCCs and seven health coaches, our skillsets include motivational interviewing, brief action planning, disease self-management and much more. We recognize the health system can be complex, so we are there to help patients overcome barriers existing around access, transportation, medication, and social services. Our goal is to guide patients to a healthier life.
