Cooper Holds First Population Health Academy Program on September 23

All employees and families are invited to attend!

The Population Health Academy is a series of eight weekly educational programs devoted to current topics regarding population health and accountable care. It was created to provide members of our Cooper family and the medical community with insights into the dramatic transformation taking place in health care today.

The sessions are free and open to Cooper employees, professional colleagues and those interested in population health. Participants are not required to attend all of the sessions and can register for sessions individually.

The first session takes place on Monday, September 23, 2013, at 5 p.m. in the auditorium of the Cooper Medical School of Rowan University on the Cooper Health Sciences Campus in Camden. It features John F. Robertson, MD, Medical Director of Cooper’s Employee Centered Medical Home.

During the session, Dr. Robertson will speak about the benefits and challenges of the Patient Centered Medical Home model and coordinating care for patients with complex medical conditions.

“The Academy will provide our physicians and clinical staff with exposure to topics centered on the future of health care,” said Anthony J. Mazzarelli, MD, Medical Director of the Population Health Institute. “We’re building a platform for transformation at Cooper and it starts with the engagement of our staff and the medical community.”

The Population Health Academy is an initiative of Cooper’s Population Health Institute and is co-sponsored by the Urban Health Institute and the Cooper Medical School of Rowan University.

The Employee Centered Medical Home, which opened in October of 2012, is one of the programs under way at Cooper designed to educate consumers and clinicians in population health and to access new care delivery models.
