Be a Partner for Patient Safety by Participating in the Patient Safety Culture Survey

It is important that we continually improve upon Culture of Patient Safety to ensure the highest safety standards to our patients. As a member of the Cooper team, you may be selected to take the Agency for Healthcare Research and Quality (AHRQ) Hospital Survey on Patient Safety Culture. If chosen, you will receive an email to participate in this important patient safety survey. The survey will take place July 7, 2014 through July 25, 2014.

The survey is web-based and is being administered by ECRI Institute, an independent, nonprofit organization dedicated to improving patient safety. When you receive an email with a link to the survey please complete it as soon as possible. Your individual responses are confidential. If you wish to provide feedback, please use the comment box below each question. Some of the questions may not apply to you so you can skip these questions. If you work in more than one location/unit, answer unit-based questions according to the area where you spend the most time and the remaining questions considering all areas where you work.

Please become our Partner for Patient Safety by completing this important survey no later than July 25, 2014. You should allow 20-30 minutes to take the survey. Results will be shared with you in September 2014.

A second survey with focus on the Physician Practices will be conducted at a later date.
