Continuing Education Program for Physicians: Heart Failure Readmission Reduction

Physicians are invited to attend the next Quality Improvement and Patient Initiatives Series on Friday, March 13, 2015, to learn about heart failure readmission reduction.

The program will be held from noon to 1 p.m. in the Roberts Pavilion, 1014 Conference Room.

The presenters are Perry J. Weinstock, MD, FACC, Director of the Cooper Heart Institute, Head of the Division of Cardiovascular Disease; and Nancy Street, MS, MBA, PMP, LSSBB, Director of the Center for Population Health.

Successful completion of one hour of a continuing medical education (CME) course recognized by the American Medical Association, the American Osteopathic Association, or the American Podiatric Medical Association is equal to one hour of continuing education for New Jersey nursing license renewal (New Jersey Board of Nursing 13:37-5.3).

The learner objective for this series is to provide physicians with actual process and performance improvement activities utilizing standard methodologies. The goal is to ensure that providers have an opportunity to understand various operational and clinical challenges within the organization and the interdisciplinary approach to continuous improvement. Projects will be presented by clinicians and administrators utilizing DMAIC methodology (Define-Measure-Analyze-Improve-Control).

The intended audience for this series includes primary care physicians and all specialists. All other health professionals including fellows and residents may also benefit from participating in this educational activity.
