OPEN ENROLLMENT 2016 – October 27 to November 10, 2015

Open Enrollment begins Tuesday, October 27, 2015, and will end on Tuesday, November 10, 2015. In the next few days, you will be receiving Open Enrollment materials which will be mailed to your home. For detailed plan information, the 2016 Benefits Highlights Booklet is available on Employee Self-Service. Click on the “2016 Benefits Handbook” link. It will also be provided at the Annual Benefits Fair.

Your Action Is Required During Open Enrollment. Current and new participants must elect a Medical Plan or Waive coverage online during Open Enrollment in order to have coverage or drop coverage for 2016. In addition, you must attest to being a Non-Tobacco or Tobacco user. If you do not enroll or waive medical coverage, you will be defaulted in the Cooper Core Plan–Employee Only coverage and will be charged the full rate for tobacco users, effective January 1, 2016.

This year, your confirmation statement will not be mailed to your home. You must print the confirmation statement at the time you complete your open enrollment elections.

Come join us at the Annual Benefits Fair on Wednesday, October 28, 2015, from 7 a.m. to 4:30 p.m. in the Roberts Pavilion 1014 conference room. Human Resources and representatives from each of our Plan’s vendors will be available to help you with any questions you may have. For your convenience, employees will also be able to receive a flu shot during the Hospital Benefits Fair.
