OPEN ENROLLMENT REMINDER – October 27 to November 10, 2015

Your action is required during open enrollment. If you have not enrolled or re-enrolled, you must take action!

All benefits-eligible employees are required to re-enroll in the medical plan or waive coverage in 2016, and also attest to being a non-tobacco or tobacco user.

If you do not enroll or waive medical coverage during Open Enrollment, you will be defaulted in the Cooper Core Plan – Employee Only coverage and will be charged the full rate for tobacco users effective January 1, 2016.

This year, your confirmation statement will not be mailed to your home. You must print the confirmation statement at the time you complete your open enrollment elections.
