Free Flu Vaccinations – January 4 and January 7, 2016

December 1, 2015, was the deadline for all Cooper employees to be in compliance with the Cooper vaccination policy. By this date, employees should have had documentation of having received their influenza vaccination for the 2015-2016 flu season or should have signed a declination form stating that they will not be receiving the vaccine. Employees should have the appropriate color sticker for the 2015-2016 flu season on their identification badge reflecting their status.

For those employees who have yet to be vaccinated or sign the declination form, you have until Friday, January 8, 2016, to complete the requirement. Any employee not in compliance by January 8, 2016, will be suspended. Those who do not comply with the requirement within seven days of suspension will be terminated.

We are again offering free flu vaccinations in the hospital cafeteria on:

  • Monday, January 4, 2016, from 7 a.m. to 9:30 a.m.
  • Thursday, January 7, 2016, from 7 a.m. to 9:30 a.m.

You may also receive your free flu vaccine at WorkNet, 300 Broadway, #101, Camden, NJ 08103.

For those employees who will not be receiving the vaccine, please print, complete and sign the declination form and return it to WorkNet.
