Have You Taken the Patient Safety Survey Yet? September 16, 2016, Is Your Last Chance!

If you, as a Cooper employee, have been chosen to take a Hospital Survey on Patient Safety Culture, please do so as soon as possible. To date, only 47 percent of employees who received the survey have completed it. Be heard. Help us improve. Your feedback will assist us with action plans for improvement, making Cooper a safer place for our patients.

One of our goals at Cooper University Health Care is to create a culture of patient safety. For this goal, we have elected to use the Agency for Healthcare Research and Quality’s (AHRQ) well validated and tested Hospital Survey on Patient Safety Culture. As a Cooper employee, you may have been chosen to take the survey. If so, you would have received an email to participate in the survey between the dates of July 25, 2016, through September 16, 2016.

If you are unable to locate the survey or have any questions regarding the survey, please contact Linda Futtersak, ECRI Institute Survey Coordinator, at 610.825.6000 ext. 5418 during the business hours of Monday through Friday from 9 a.m. to 5 p.m., EST, or email your questions to lfuttersak@ecri.org.

The survey is confidential and is being administered by ECRI Institute, an independent, nonprofit organization dedicated to improving patient safety. If you receive an email from ECRI, the email will contain a link to the web-based survey. The survey should take approximately 30 minutes and we ask that you please read the instructions in the email message carefully and complete the survey as soon as possible.

Some of the survey questions may not apply to you. Skip those questions. If you work in more than one location or unit, answer unit-based questions according to the area where you spend the most time.

It is important that we hear from you to evaluate the patient safety culture at Cooper University Health Care. As a result of our last survey, many important improvements were implemented to provide safer care for our patients. Further improvements and initiatives depend on you!
