The Urban Health Institute opened the first Cooper Advanced Care Center (CACC) four years ago at Sheridan Pavilion (Three Cooper Plaza), Camden, with an exciting new patient care model featuring care coordination and LPN-led protocols that have reduced costs and have improved patient outcomes, increased patient participation, and enhanced patient satisfaction. Based on these successes and more, the Urban Health Institute is pleased to announce the expansion of their program and the opening of their first satellite office at the Kroc Center at 1865 Harrison Avenue in Camden. Congratulations to Steven T. Kaufman, MD, Medical Director, and his team on the growing success of the Cooper Urban Health Institute.
