Final Reminder: Wellness Incentive Program Ends Today!

Cooper is pleased to offer a Wellness Incentive Program to all employees enrolled in the Cooper Basic, Core and Standard PPO Plans. To be eligible, you must complete all three steps listed between from November 1, 2016, and October 31, 2017. Upon successful completion, you will not be charged for one bi-weekly Medical Plan payroll deduction in December 2017.

Step 1.  Complete the annual physical.

Step 2.  Attest to non-tobacco use during 2017.

Step 3.  Complete the Online Personal Health Risk Assessment via the AmeriHealth Administrators web site by October 31, 2017.
