Last Chance to Enroll!

All benefits-eligible employees are required to enroll, re-enroll, or waive coverage by Wednesday, November 29, 2017. If you do not take action, you will be placed into the Cooper Core Plan, Employee-Only coverage and will not receive the non-tobacco discount.

If you do not take the required action and are currently covering dependents on your Cooper health plan, they will no longer be covered for 2018.

To choose your benefits, visit Employee Self-Service (ESS) on the portal by clicking here.

You can view the 2018 Benefits Highlight booklet on ESS. Log onto ESS and click on the OPEN ENROLLMENT link to view all materials.

Remember to print your enrollment confirmation statement at the time you complete your elections via ESS. The enrollment confirmation statement will not be mailed to your home.
