The Center for Population Health is excited to receive nominations for its inaugural Quarterly Award. The recognition will be awarded to an individual that best exemplifies the values of Innovation, Patient Empowerment, Commitment to Excellence, and Compassionate Care. Population Health includes multidisciplinary collaboration (e.g., providers, IT, Informatics, etc.) and encourages nominations across the organizational care continuum. Nominations will be accepted through July 17, 2020, and a winner will be announced by July 31, 2020.
