Decoding Population Health’s Alphabet Soup

You may have seen many different acronyms in relation to the Population Health Department, so we are here to decipher them!

PCC – Patient Care Coordinator: Nurse care coordination for high-risk populations discharged from local hospitals and/or in Cooper’s primary care setting.

HA – Health Advisor: Complex care coordination for at-risk populations in Cooper’s primary care setting.

CHW – Community Health Worker: Community outreach with Camden and Gloucester City residents to address social determinants of health and social needs through goal setting and connection to local resources.

AHC (screeners) – Accountable Health Communities: Screen patients in Cooper’s Emergency Department for social determinant of health needs and connect patients to local resources.

CPHAH – Cooper Population Health at Home: Remote patient monitoring by an advanced practice nurse and emotional support by a licensed clinical social worker in the patient’s home.

ACO – Accountable Care Organization: A hospital system or physician practice group that participates in Medicare’s shared savings program where the Centers for Medicare and Medicaid Services (CMS) compensates for both quality metric performance and cost utilization.
