Remote Patient Monitoring Program Delivers Care to Patients’ Homes

The Remote Patient Monitoring (RPM) program, led by the Innovative Health Delivery team, is one of a multitude of programs that bring health care directly to where our patients live, whether a home, apartment, or a post-acute care facility.

Patients with chronic medical conditions who are discharged from a hospital admission are sent home with a personalized monitoring kit that may include a blood pressure monitor, thermometer, pulse oximeter, scale, and/or glucometer. Information obtained from the screening tools is transmitted to the patient’s supplied smart phone or tablet, and then to their electronic health record in a safe, secure manner for the RPM team to review. The transmitted data helps the care team make better decisions about a patient’s medical care while allowing the patient to remain in their post-acute care location.

The RPM program launched earlier this year. Currently, the RPM program is supporting patients with a heart failure diagnosis or at post-COVID-19 admission. Soon, the program will expand to patients with COPD.
