Deadline Approaching: 2023 Flu Vaccination Program

The 2023 Flu Vaccination Program is ending soon. The deadline to receive the vaccination or submit an exemption form is Thursday, December 7, 2023Click here for the schedule.

New Jersey law mandates individuals who work in a hospital, nursing home, or home health care agency must receive an influenza vaccination. The state law allows for limited medical exemptions. If you are seeking a medical or religious exemption, please fill out the medical exemption form here and return it to Employee Health at flu@cooperhealth.edu by Thursday, December 7, 2023. The form will only be processed if you return it to Employee Health – please do not submit it to your manager.

If you are seeking a personal exemption from the flu vaccination, and you do not work in the hospital or a licensed facility, please fill out this form and return it to Employee Health at flu@cooperhealth.edu by Thursday, December 7, 2023. Once flu season is officially declared, team members who have received an approved exemption and have not received a flu vaccination must wear a mask in all Cooper facilities until flu season has ended.

Additional information:

All Cooper team members, physicians, residents, students, and volunteers may receive the flu vaccination from an Employee Health nurse at any of the times and locations listed on the schedule.

  • This year, Cooper will administer the quadrivalent flu vaccine, which is designed to protect against four different strains of flu virus, including two influenza A viruses and two influenza B viruses.
  • Team members who receive the vaccination at a private physician office or a public clinic must submit proof of immunization via email to Employee Health at flu@cooperhealth.edu. This information will onlybe processed if you return it to Employee Health – please do not submit it to your manager. All proof of vaccination must have:
    • Your name.
    • Your birth date.
    • The date the vaccine was administered.
    • Manufacturer of vaccine.
    • The lot number of the vaccine.
    • The expiration date.
    • The location of the injection site: right arm or left arm.
  • Team members who are on a leave of absence will receive a letter at home with instructions on how to receive their flu vaccination.
  • Team members can view their flu vaccination status in iCooper under “My Health.”
  • Managers: Please be sure your team member roster is current.
