Share Feedback for the South Jersey Health Collaborative

Every three years, Cooper participates in a Community Health Needs Assessment to better understand the health-related needs of our community and inform our future projects and initiatives. Cooper, alongside Virtua, Jefferson, and the Burlington, Camden, and Gloucester County Health Departments, collaborates on this project and seeks your feedback as one of the many steps in this process. As a health collaborative, we are asking key community leaders (e.g., Cooper team members) to share their thoughts about strengths, barriers, and opportunities for improving health and social determinants of health across South Jersey, as well as to shed light on other organizational and community priorities. Your response will help us identify and align priority areas for collective action.

Click here to take the survey. The survey is open through January 17, 2025, and will take approximately 10 minutes to complete. All responses are anonymous.

For additional questions or ways to get involved with the Community Health Needs Assessment process, please contact Danielle Santiago-Roach or Max Kursh.

Thank you for helping inform and influence future initiatives at Cooper and within the community we serve.
