We are approaching another compliance season for our team members here at Cooper. Employee Health will be actively working with team members across Cooper in the coming weeks to complete their mandatory annual tuberculin risk assessment and screening forms. Please note that this requirement applies to all team members, regardless of job title or position.
The Employee Health team will be available during the following dates and times at the CCBO:
- Tuesday, March 25, from 1 p.m. to 4 p.m., Conference Room 201, CCBO
- Wednesday, March 26, from 8 a.m. to noon, Conference Room 201, CCBO *room change*
- Monday, March 31, from 1:30 p.m. to 3 p.m., Auditorium 1, CCBO
- Wednesday, April 9, from 8:30 a.m. to 11 a.m., Auditorium 1, CCBO
Attention CCBO remote team members:
To complete your tuberculin screening, please follow these steps:
- Download: Click here to download the tuberculin screening assessment form.
- Complete: Fill out the form electronically and save the new copy.
- Submit: Email the completed form to EmployeeHealthNurse@cooperhealth.edu.
